What should I expect during my first visit?

To make your visit with us as pleasant as possible, please take a moment to learn more about our office and its procedures. During your first visit you will be asked to fill out a few forms that will enable us to diagnose and treat you in the best manner possible. Please make sure that you fill the forms out completely and ask a staff member if any help is needed. We will perform an oral exam and take a set of x-rays for your records and to help us establish the best manner of treatment for your dental needs. If you have any questions, please don't hesitate to ask us. We appreciate the confidence you have placed in us and look forward to working with you.

How do I schedule my appointment?

To schedule an appointment, simply call our office: (904) 240-4144.

What if I need to cancel or re-schedule my appointment?

Our appointments are scheduled specifically for each patient. We ask that you notify us at least 48 hours in advance if you are unable to attend.

How much will it cost?

The prices of our services vary depending upon which services are required for your personal needs. We'll make every effort to inform you of the cost of your dental work prior to treatment.

How can I pay for my services?

For your convenience, we accept Master Card, Visa, Discover and, with proper ID, your personal check. We also offer flexible payment plans through an outside finance company which can be applied for and verified while you wait. We also accept a variety of traditional dental insurance plans. If you have insurance, please call our office in advance and bring your plan information on your first visit.

Patient Forms

The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time, you will save a significant amount of time during your visit. Please be prepared to provide this information to our office along with your current medical insurance information. If you have any questions when filling out these forms, please do the best you can and our staff will assist you with your questions on the day of your visit.
